I have seen pro players perform like crap, i have seen crap players perform good, you can't decide a person doesn't deserve their MMR based on a few games watched.
Well, maybe you can't Havoc. But I believe I can tell who has skill and who does not from watching a few games. I don't simply watch as if I'm casually seeing a dota game. I'm looking for specific things. Do they check items? how fast? How frequently? How fast do they react to attacks from fog/invis? How well do they use active items and abilities during chaotic fights? What is their situational awareness? Do they adapt their build to the game? How precisely can they lasthit? Do they require tons of animation canceling? How many misclicks are they doing? How tight is their movement control? How smooth is their mouse movement? Are they aware of the enemies vision? etc.
Based on these wave is about a top 1% player, that translates to 4k MMR if he did not cheat so much. In my opinion if he simply played the game like everyone else he would end up near 4k MMR based on watching his actual skill playing dota.

As much as I like some of your previous posts Relentless, you're off on a completely wrong track here, wave might have his issues but he's not an account buyer, I've met him a couple of times in MM and he's usually pretty tolerable.
There is no combination of things that derives a good player, there are many people that are higher that me whom I don't think are that much better or even good when I see them play but that could also be because what I deem to be skill might not be what people in that particular bracket look for the most.
In fact I have found with most players what they lack is execution and mechanical skills. All the things you mentioned above are good if you want to improve on a fundamental level if you're between 2-3k which will help you advance in terms of how to go through the motions of a good player, it doesn't however make you a good player.
All of the listed above I see a lot of low skill players doing....so why are they there? I have also watched games, streams, etc from the top players and seen them do none of the above either.
As a support player my job in games is actually quite hard, pinging ganks, calling wards, placing wards, counterganking, ganking, calling items, buying detection etc. I do all those things so the cores don't have to. The game is all about the team and I would prefer my mids have more focus on what they're doing then looking around the map, checking items and looking out for ganks.
Its (IMO) a pretty archaic way to view the game as a lot of mid's aren't even worried cause they have the confidence that they can out outmaneuver their opponents if ganked as is often the case with good mid players.
I see pro players, mid, offlane, even the carry (between lasthits) checking what is happening around the map all the time. It's not an outdated concept. While supports may have time to stop and consider the situation, everyone still has to keep tabs on things.
There is much more to dota 2 than mechanics, but solo ranked MMR is determined largely by mechanics. It is not hard to see the difference between a 4k player and a 5k player in mechanical skills if you look at their mechanical skills. That difference in speed and accuracy is the reason one is 1000 points higher. Game knowledge and cooperation make a big difference if you play with a 5 stack, the same team, regularly. But in solo ranked matching MMR translates clearly to mechanical skill. So when you see someone losing a matchup mid their hero normally wins easily...losing it because they can't keep up with the speed of the other player and are making tons of misclicks trying to do it... when you see that you know they are just out of their league.
Usually I see this and it connects to their dotabuff stats which show that they have almost never played that hero before, and that is why they can't play it at a certain MMR level. But in this case I see a player using heroes that he supposedly has racked up amazing stats --- yet he still plays them like he lacks the talent to use them at MMR level. The clear conclusion is that someone else got those stats, then the weaker player took over the account.
