Vsync prevents 'tearing' of the screen.
Generally there are no problems with leaving it on. Some elitists want to maintain 120 fps but there is no notable difference between 60fps and 120 fps.
I have tearing but it's not that bad imo. It only shows in the fog or black places.
Without VSync I have more fps.
Theres a huge difference between 60fps and 120fps when you play FPS/TPV games. In a game like dota, 60fps is plenty smooth as it is.
The fact you're advocating 120fps without even asking the Hz of OP's monitor shows you don't have a clue regarding this matter except for 'higher = better'.
It limits the fps to the refresh rate of your monitor.
If you have a stable fps, without any spikes in team fights, you should enable it. Even if you don't feel any difference, it makes the game easier on the eyes, especially on big screens.
Also, make sure to enable triple buffering as well, it's not in the options but can be enabled via the console using mat_triplebuffered 1

This is how it works boys!
(Fps) Framerate meaning the frames produced.
(hz) Refreshrate meaning the number of times per second your Display will UPDATE with new information.
Vertical sync = removes some tearing and limits FPS to HZ + and also gives input delay ( this is why pros dont use it ).
120 and 60 is a huge difference.
enormous, tremendous, extremely noticeable difference.

Unless you are fighter jet pilot it's impossible for human eye to notice difference between 60 and 120 FPS.
